Registration Date

There are a few things you will need to do so your player's registration is complete. All paperwork must be turned in to the Board no later than August 1, 2018 to complete your registration.
1. Player Contract - Please fill it out completely.
2. Physical form - Please print the physical form and fill out the first page. Have your doctor fill out the second page. Make sure your doctor has signed and dated the form in 2018.
3. Birth Certificate - Please bring your child's ORIGINAL Birth Certificate. We will makes copies
4. Report Card - When final report cards are issued in June
